Person-centred support

We recognise that if you have more complex care needs and a combination of health, social and behavioural challenges you may require more tailored support. We focus on strengths and provide specialist support for chronic and long-term health conditions, and for people who may require extra assistance to manage symptoms and day to day activities.

We can offer either a building-based or community-based service, providing quality support and care outcomes where you are facilitated to lead active and fulfilled lives with the level of independence that is right for you.
